created_at is a Unix timestamp in milliseconds, so convert it before showing it to people.

Can I page through a large review set?

Yes. Use start_page together with max_items to continue a pull.

Do I pay for lookups that come back empty?

No. You are charged per result, so a request that finds nothing costs nothing. Testing this against a messy list is free to get wrong.
