What do the sort values mean?

1 is lowest price, 2 highest price, 3 best selling, 4 best rating and 5 most reviews.

Are sponsored results flagged?

Yes. Each result carries an is_sponsored flag, so you can filter them out.

Do I pay for lookups that come back empty?

No. You are charged per result, so a request that finds nothing costs nothing. Testing this against a messy list is free to get wrong.
